I am looking from a glass half-full perspective...
1. McCargo isn't healthy enough to pass a physical and the Bills know it. Buffalo then attempts to slip him to another team in a trade that brings us a 4th rounder.
2. McGargo isn't healthy enough to pass a physical and the Bills know it. They figure maybe he thinks he's in good enough shape to play and is just pissed off at the team and is expecting to go elsewhere or even force a trade since he is so low on the depth chart. So the Bills (knowing he'll fail the physical) trade him, call him out in the media, etc... knowing full well that the Colts will deny the trade and send him back to Buffalo.
Now the whole NFL knows this kid is lazy, and now McCargo knows that no other team is willing to take a chance on him... so what happens? McCargo realizes that his meal ticket resides solely with the Bills and perhaps he become motivated to prove everyone wrong.
